The first, from Germany, followed 505 overweight adults with MASLD and found that weekly oral 14,000 IU of vitamin D3, combined with lifestyle guidance, significantly reduced liver fat content. The second study, published July 30 in Annals of Medicine, took the opposite population: 211 normal-weight adults with MASLD compared with nearly 4,000 healthy controls. Even at a healthy BMI, low vitamin D levels were linked to higher MASLD prevalence, worse steatosis, and higher fibrosis risk. Vitamin D3 supplementation in that group improved blood sugar, triglycerides, and ALT/AST - the key liver enzymes.
Why vitamin D and the liver are connected
The mechanism makes biological sense and is worth understanding: the liver is the main organ that converts vitamin D into its active form, and when liver function is impaired, that conversion efficiency drops. At the same time, excess fat tissue can sequester vitamin D, leaving less circulating in the blood.
